Let’s build the future of mental health care together!</p><p>We are seeking a highly organized and motivated individual to join our team as a Facilitated Telehealth Coordinator. As a Facilitated Telehealth Coordinator, you will serve as the in-person liaison between the patient and our virtual care team. This position offers an exciting opportunity to contribute to the advancement of healthcare technology and improve access to care for our patients</p><p><strong>WHAT IS YOUR SUPERPOWER?</strong></p><ul><li>Facilitate communication between providers and their patients including assisting patients use of technology to conduct virtual visits with other providers on the team</li><li>Assess environment regarding safety concerns or potential hazards</li><li>Communicate with the patient care team regarding any patient questions and needs such as medications, Community Based Organizations.</li><li>Coordinate telehealth services and ensure seamless delivery of care to patients in various settings: home, clinic, living communities (ALFs, etc.)</li><li>Provide education and support to patients and caregivers on how to use telehealth technology effectively</li><li>Maintain accurate and up-to-date records of telehealth appointments and patient data</li><li>Collaborate with IT and technical support teams to resolve any telehealth system issues</li><li>Conduct training sessions to educate healthcare staff on telehealth best practices and workflows</li><li>Collaborate with cross-functional teams to identify technology needs and opportunities for improvement</li><li>Troubleshoot and resolve technology issues in a timely manner to minimize downtime.</li><li>Occasionally, on an as needed basis, conduct in-person outreach to doctors offices, hospitals, and patients' homes to educate potential patients and caregivers about Author Health's mission and service.</li></ul><p><strong>WHAT WE ARE SEEKING:</strong></p><ul><li>Comfortable working with the senior population and those with behavioral health conditions</li><li>Proven experience working in a healthcare setting or similar role</li><li>Knowledge of telehealth technology and software</li><li>Excellent organizational and time management skills</li><li>Strong attention to detail and ability to multitask</li><li>Excellent communication and interpersonal skills</li><li>Excellent problem-solving and communication skills.</li><li>Ability to work independently and collaborate effectively with team members</li><li>Proactive and adaptable to change in a fast-paced environment</li><li>Possession of driver’s license and car </li><li>Preferred Bilingual (Spanish-English)</li><li>Willingness to travel up to 100 miles round trip per day</li></ul><p><strong>PREFERRED:</strong></p><ul><li>At least 1 year of experience directly related to the duties of the position</li><li>Passion for, and experience working with, patients with mental health conditions</li><li>Passion for, and experience working with, seniors</li></ul><p>This role does require walking to and from patients’ homes in all weather conditions. This role demands a high level of flexibility, independence, and reliable transportation. In addition to in-person work, employees will occasionally work from home, utilizing virtual tools and software to manage tasks, communicate with team members, and complete administrative duties.
